The Targeted Case Manager will be responsible to assist the individual with obtaining needed access to housing, medical, social, educational and other identified services.
Qualifications Bachelor's degree in a behavioral science (including psychology, sociology, social work, family studies, human services, counseling, or another approved human service degree).
A minimum of one (1) year of full-time employment working directly adults in a human service setting; in the field of Substance Use; Successful completion of Targeted Case Management training approved by the Kentucky Department for Behavioral Health, Developmental and Intellectual Disabilities (KBHDID) and completion of recertification requirements approved by KBHDID every three year Ability to work in a fast paced, dynamic environment.
Valid Kentucky driver's license.
Responsibilities To be well versed on programs available, policy procedure and rules so you can inform our clients and their families as much as possible before and during entry and be able to sell the program to prospective clients vs.

This requires you to be proficient in knowing as much as possible on all other Kentucky programs.
Assisting the client in gaining access to needed medical, social, educational, or other needed services and supports, including:
(a) Assessment of the client's medical, social, and functional status and identification of the client's strengths and needs.
(b)Arranging for service delivery from the client's chosen provider to ensure access to required services.
(c) Facilitating access to needed services by explaining the need for and importance of services in relation to the client's condition.
(d) Facilitating access, quality, and delivery of necessary services; and (e) Preparation and maintenance of case record documentation to include care plans, forms, reports, and narratives as appropriate Responsible for conducting a comprehensive assessment and a periodic reassessment of an individual's strengths and needs; and Responsible for assisting an individual to gain access to identified medical, social, educational, and other service needs.
Provide case management services for a maximum caseload of 25 Individual's with SUD Represent agency programs, policies and procedures accurately in the community and with other social service agencies.
Ensure that all documentation complies with KY Medicaid and accreditation standards.
Participate in bi-monthly supervision with TCM billing Supervisor.
Participate in monthly staff meetings and lead in trainings/discussions as assigned.
Complete required 10-hour SUD training to maintain specialty certifications.
Maintain certification requirements for TCM pursuant Kentucky Board regulations.
Any employee wishing to resign is required to furnish a written notice of such intention addressed to the employee's immediate supervisor.
All employees are entitled to all earned, unpaid salary.
All termination pay will be distributed to the employee on the next payday following the termination.
This position requires 30 days resignation notice.
